Our Community Health Workers Program offers one-on-one support to help you navigate the resources and services you need at no-cost.
Our team provides:

Personalized one-on-one care coordination
-
Assessments to identify and prioritize areas of concern and/or improvement
- Connects you to trusted community resources and campaigns such as:
-Employment and training initiatives
-Emergency planning
-Health education and coaching
-Food security
-Early childhood education or family supports
-Access to legal and general information
-Language and literacy supports
-Access to primary care, understanding your insurance coverage and increasing your health literacy
- Offers assistance with applications for public benefit programs such as:
-Medi-Cal
-CalFresh
-Utility assistance program like REACH
-Expungement application assistance and guidance and many more!
We also soon provide housing support services for Medi-Cal recipients.
